An unusual case of symptomatic partial rupture of the extensor carpi ulnaris (ECU) tendon after trauma is described. The patient was successfully treated by surgical removal of a prominent ulnar ridge, debridement of the ECU tendon, and ECU subsheath reconstruction. Extensor carpi ulnaris subluxation and partial rupture should be considered in the differential diagnosis of a patient with chronic ulnar wrist pain.
