Abstract

OBJECTIVE

This study aimed to observe the diagnostic value of cranial magnetic resonance imaging (MRI) in patients with vascular dementia induced by ischemic stroke.

METHODS

The experiment was designed according to the randomized control principle. Two hundred and eighty patients with ischemic stroke who were admitted to Gucheng County Hospital between June 2019 and June 2021 were selected as research subjects. Patients without vascular dementia after stroke were included in the control group, and patients with vascular dementia after stroke were included in the observation group. The cranial MRI was performed in both groups.

RESULT

Proportions of patients with large and moderate infarct lesions in brain tissues were significantly higher in the observation group than the control group. The data variation of relevant MRI detection indicators of the observation group was more obvious than that of the control group (P<0.05). The mini-mental state examination (MMSE) and Montreal cognitive assessment (MoCA) scores of the observation group were lower than those of the control group (P<0.05), but the HIS score was higher (P>0.05). Patients with changes in brain morphology were more in the observation group than the control group (P<0.05).

CONCLUSION

Patients with vascular dementia induced by ischemic stroke are characterized by cortical atrophy, widening of the cerebral sulcus, large infarct lesion area and sparse cerebral white matter. Cranial MRI can effectively identify these features. The application of cranial MRI has some clinical values for early treatment and prognostic assessment.

目的

本研究旨在观察头颅磁共振成像(MRI)对缺血性脑卒中所致血管性痴呆患者的诊断价值。

方法

实验按照随机对照原则设计。选取2019年6月至2021年6月期间在固城县医院收治的280例缺血性脑卒中患者作为研究对象。脑卒中后无血管性痴呆的患者纳入对照组,脑卒中后有血管性痴呆的患者纳入观察组。两组均进行头颅MRI检查。

结果

观察组脑组织中大面积和中度梗死灶患者的比例显著高于对照组。观察组相关MRI检测指标的数据变化比对照组更明显(P<0.05)。观察组的简易精神状态检查表(MMSE)和蒙特利尔认知评估量表(MoCA)评分低于对照组(P<0.05),但HIS评分更高(P>0.05)。观察组脑形态改变的患者比对照组更多(P<0.05)。

结论

缺血性脑卒中所致血管性痴呆患者的特征为皮质萎缩、脑沟增宽、梗死灶面积大及脑白质稀疏。头颅MRI能够有效识别这些特征。头颅MRI的应用对早期治疗及预后评估具有一定临床价值。
