Ultimate drug levels and, therefore, maximum drug effect in general are inversely related to the function of the organ of excretion. If organ deficit is known, dose reduction should be automatic; if a drug level is very high at an ordinary dose, the organ of excretion should be investigated. The physician must not assume that a patient is not taking drugs just because the physician himself has not prescribed them. The elderly take more over-the-counter drugs than other segments of the population and frequently go to many health care providers.
一般来说,最终药物水平以及因此产生的最大药物效应与排泄器官的功能呈负相关。如果已知排泄器官功能不足,应自动减少剂量;如果常规剂量下药物水平很高,则应检查排泄器官。医生不能仅仅因为自己没有开药就假定患者没有服药。老年人服用非处方药的比例高于其他人群,并且经常会看多个医疗服务提供者。
