Improving health related quality of life is an important goal of aortic valve replacement. Inadequate effective orifice area of prosthesis according to the patient's body surface area may be associated with poor outcomes. In this study, we aimed to analyze impact of indexed effective orifice area (iEOA) on patients' quality of life after aortic valve replacement.
A total of 138 patients who underwent isolated aortic valve replacement were included to the study. Quality of life assessment was performed with EuroQol Group EQ-5D-5L questionnaire. Patients were divided into three groups based on iEOA (Group 1 had an iEOA of <0.65 cm2/m2 (19 patients), Group 2 had an iEOA between 0.65-0.85 cm2/m2 (71 patients), and Group 3 had an iEOA of >0.85 cm2/m2). Mean EQ-5D-5L scores were compared among the groups statistically.
Mean EQ-5D-5L scores were lower in Group 1 than in Groups 2 and 3 (Group 1: 0.72 ± 0.18, Group 2: 0.83 ± 0.20, and Group 3: 0.86 ± 0.9, p = 0.044 and p = 0.014). The EQ-5D-5L score was significantly lower in patients with a ≥20 mmHg transvalvular gradient than those with a <20 mmHg (0.74 ± 0.25 vs. 0.84 ± 0.18, p = 0.014).
Our results show that an iEOA <0.65 cm2/m2 is significantly associated with impaired postoperative health-related quality of life. Newer generation prostheses, transcatheter valve implantation, and root enlargement techniques should be kept in mind in preoperative planning.
提高健康相关生活质量是主动脉瓣置换的重要目标。根据患者的体表面积,人工瓣膜有效开口面积不足可能与不良预后相关。在这项研究中,我们旨在分析索引有效开口面积(iEOA)对主动脉瓣置换后患者生活质量的影响。
共纳入 138 例接受单纯主动脉瓣置换术的患者进行研究。采用 EuroQol 组 EQ-5D-5L 问卷进行生活质量评估。根据 iEOA 将患者分为三组(组 1:iEOA<0.65cm2/m2,19 例;组 2:iEOA 在 0.65-0.85cm2/m2 之间,71 例;组 3:iEOA>0.85cm2/m2,48 例)。统计比较三组间平均 EQ-5D-5L 评分。
组 1 的平均 EQ-5D-5L 评分低于组 2 和组 3(组 1:0.72±0.18,组 2:0.83±0.20,组 3:0.86±0.9,p=0.044 和 p=0.014)。跨瓣压差≥20mmHg 的患者 EQ-5D-5L 评分明显低于<20mmHg 的患者(0.74±0.25 vs. 0.84±0.18,p=0.014)。
我们的研究结果表明,iEOA<0.65cm2/m2 与术后健康相关生活质量受损显著相关。在术前规划中应考虑使用新一代瓣膜、经导管瓣膜植入术和根部扩大技术。
