To develop and validate a dynamic contrast-enhanced (DCE) MRI-based radiomics model to predict epidermal growth factor receptor (EGFR) amplification in patients with glioblastoma, isocitrate dehydrogenase (IDH) wildtype.
Patients with pathologically confirmed glioblastoma, IDH wildtype, from January 2015 to December 2020, with an EGFR amplification status, were included. Patients who did not undergo DCE or conventional brain MRI were excluded. Patients were categorized into training and test sets by a ratio of 7:3. DCE MRI data were used to generate volume transfer constant (K) and extracellular volume fraction (V) maps. K, V, and conventional MRI were then used to extract the radiomics features, from which the prediction models for EGFR amplification status were developed and validated.
A total of 190 patients (mean age, 59.9; male, 55.3%), divided into training (n = 133) and test (n = 57) sets, were enrolled. In the test set, the radiomics model using the K map exhibited the highest area under the receiver operating characteristic curve (AUROC), 0.80 (95% confidence interval [CI], 0.65-0.95). The AUROC for the V map-based and conventional MRI-based models were 0.74 (95% CI, 0.58-0.90) and 0.76 (95% CI, 0.61-0.91).
The DCE MRI-based radiomics model that predicts EGFR amplification in glioblastoma, IDH wildtype, was developed and validated. The MRI-based radiomics model using the K map has higher AUROC than conventional MRI.
开发和验证一种基于动态对比增强(DCE)MRI 的放射组学模型,以预测 IDH 野生型胶质母细胞瘤患者表皮生长因子受体(EGFR)扩增情况。
纳入 2015 年 1 月至 2020 年 12 月期间经病理证实为 IDH 野生型胶质母细胞瘤且 EGFR 扩增状态明确的患者。排除未行 DCE 或常规脑 MRI 检查的患者。采用 7:3 的比例将患者分为训练集和测试集。利用 DCE MRI 数据生成容积转移常数(K)和细胞外容积分数(V)图。从 K、V 和常规 MRI 中提取放射组学特征,建立并验证 EGFR 扩增状态的预测模型。
共纳入 190 例患者(平均年龄 59.9 岁,男性占 55.3%),分为训练集(n=133)和测试集(n=57)。在测试集中,基于 K 图的放射组学模型的受试者工作特征曲线下面积(AUROC)最高,为 0.80(95%置信区间[CI],0.65-0.95)。基于 V 图和常规 MRI 的模型的 AUROC 分别为 0.74(95% CI,0.58-0.90)和 0.76(95% CI,0.61-0.91)。
