The aim of the present study was to investigate the influence of comorbid oppositional defiant disorder (ODD) on clinical features and neuropsychological profiles of children with attention-deficit/hyperactivity disorder (ADHD).
We divided the participants into three groups: the ADHD with ODD (ADHD/ODD) (n=36), ADHD without ODD (ADHD/noODD) (n=307), and control groups (n=128). Parents of the participants completed the ADHD Rating Scale, Social Responsiveness Scale (SRS), Korean Personality Rating Scale for Children (K-PRC), and 10-item mania scale from the Parent General Behavior Inventory (P-GBI-10M). Neuropsychological profiles were assessed using the Advanced Test of Attention (ATA), Children's Color Trails Test, and Stroop Color and Word Test.
The ADHD/ODD group had more ADHD symptoms and functional impairments in relationships with teachers and peers, and self-esteem than the ADHD/noODD group. The ADHD/ODD group scored higher in Social Communication (p<0.001) and Autistic Mannerisms (p<0.001) subscales of SRS, P-GBI-10M (p<0.001), and Delinquency (p<0.001) and Psychosis (p<0.001) subscales of K-PRC than the ADHD/noODD group. Commission Errors (p<0.001) and Response-Time Variability (p<0.001) in Visual ATA and Commission Errors (p<0.001) in Auditory ATA were significantly higher in the ADHD/ODD group than in the ADHD/noODD group.
The present study suggests that patients with ADHD with ODD experience more ADHD symptoms and neuropsychological deficits than those with ADHD without ODD. These results also imply that comorbid ODD is associated with greater social impairment and emotional dysregulation.
本研究旨在调查对立违抗障碍(ODD)共病对注意力缺陷多动障碍(ADHD)儿童临床特征及神经心理学特征的影响。
我们将参与者分为三组:ADHD伴ODD(ADHD/ODD)组(n = 36)、ADHD不伴ODD(ADHD/noODD)组(n = 307)和对照组(n = 128)。参与者的父母完成了ADHD评定量表、社会反应量表(SRS)、韩国儿童人格评定量表(K-PRC)以及父母一般行为量表中的10项躁狂量表(P-GBI-10M)。使用注意力高级测试(ATA)、儿童颜色连线测试和Stroop颜色与文字测试对神经心理学特征进行评估。
与ADHD/noODD组相比,ADHD/ODD组具有更多的ADHD症状,在与教师和同伴的关系以及自尊方面存在更多功能损害。ADHD/ODD组在SRS的社会沟通(p<0.001)和自闭症行为(p<0.001)子量表、P-GBI-10M(p<0.001)以及K-PRC的犯罪行为(p<0.001)和精神病(p<0.001)子量表上的得分高于ADHD/noODD组。ADHD/ODD组在视觉ATA中的 commission错误(p<0.001)和反应时间变异性(p<0.001)以及听觉ATA中的commission错误(p<0.001)显著高于ADHD/noODD组。
本研究表明,ADHD伴ODD的患者比ADHD不伴ODD的患者经历更多的ADHD症状和神经心理学缺陷。这些结果还意味着共病ODD与更严重的社会损害和情绪失调有关。
