Abstract

OBJECTIVES

Our study aimed to elucidate the possible relationship between endogenous circulating testosterone and the beginning and development of stress urinary incontinence (SUI) in postmenopausal women.

PATIENTS AND METHODS

The clinical data of female patients with SUI who underwent surgery at our hospital from January 2014 to February 2023 and healthy female volunteers recruited during the same period were retrospectively analyzed according to age and body mass index (BMI). Venous blood samples were taken from all subjects, and levels of estradiol, luteinizing hormone, prolactin, follicle-stimulating hormone, progesterone, and testosterone were measured by radioimmunoassay. After adjusting for age, BMI, hypertension, mode of delivery, hysterectomy, and profession, multiple logistic regression analysis was used to determine the relationship between SUI and serum testosterone levels in postmenopausal women.

RESULTS

Serum testosterone levels were significantly lower in women with SUI than in healthy control women (0.92 ± 0.67 vs. 1.28 ± 1.10; P < 0.05). Further comparison of testosterone levels between postmenopausal SUI women and healthy postmenopausal women in postmenopausal subjects revealed that testosterone levels were significantly lower in postmenopausal SUI women than in healthy postmenopausal women (0.84 ± 0.64 vs. 1.23 ± 1.10; P < 0.05). This difference in testosterone levels remained significant after controlling for age, BMI, hypertension, mode of delivery, hysterectomy, and profession in postmenopausal women.

CONCLUSION

The results of the present study indicate that low levels of serum testosterone are associated with an increased likelihood of stress urinary incontinence in women. Low serum testosterone levels may be a risk factor for SUI in postmenopausal women.

目的

本研究旨在阐明内源性循环睾酮与绝经后女性压力性尿失禁(SUI)的发生和发展之间可能存在的关系。

患者与方法

根据年龄和体重指数(BMI),回顾性分析了 2014 年 1 月至 2023 年 2 月在我院接受手术的 SUI 女性患者和同期招募的健康女性志愿者的临床资料。所有受试者均采集静脉血,采用放射免疫法测定雌二醇、促黄体生成素、催乳素、卵泡刺激素、孕酮和睾酮水平。在调整年龄、BMI、高血压、分娩方式、子宫切除术和职业后,采用多因素 logistic 回归分析确定绝经后女性 SUI 与血清睾酮水平的关系。

结果

SUI 组女性血清睾酮水平明显低于健康对照组(0.92±0.67 比 1.28±1.10;P<0.05)。进一步比较绝经后 SUI 女性和健康绝经后女性的睾酮水平,发现绝经后 SUI 女性的睾酮水平明显低于健康绝经后女性(0.84±0.64 比 1.23±1.10;P<0.05)。在控制了绝经后女性的年龄、BMI、高血压、分娩方式、子宫切除术和职业后,这种睾酮水平的差异仍然显著。

结论

本研究结果表明,血清睾酮水平低与女性压力性尿失禁的发生几率增加有关。低血清睾酮水平可能是绝经后女性 SUI 的一个危险因素。
