Abstract

OBJECTIVE

To test the hypothesis that moxibustion may inhibit rheumatoid arthritis (RA) synovial inflammation by regulating the expression of macrophage migration inhibitory factor (MIF)/glucocorticoids (GCs).

METHODS

Fifty male Sprague-Dawley rats were randomly divided into five groups ( 10 each): blank Control (CON) group, RA Model (RA) group, Moxibustion (MOX) group, MIF inhibitor (S,R)-3-(4-hydroxyphenyl)-4,5-dihydro-5-isoxazole acetic acid methyl ester (ISO-1) group, and Moxibustion + MIF inhibitor ISO-1 (MOX + ISO-1) group. Rats in the ISO-1 group and ISO-1 + MOX group were intraperitoneally injected with the inhibitor ISO-1. The rats in the RA group, ISO-1 group, MOX group, and ISO-1 + MOX group were injected with Freund's complete adjuvant (FCA) in the right hind footpad to establish an experimental RA rat model. In the MOX group and MOX + ISO-1 group, rats were treated with Moxa. The thickness of the footpads of the rats in each group was measured at three-time points before, after modeling and after moxibustion treatment. The contents of serum MIF, corticosterone (CORT), tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) were detected by enzyme-linked immunosorbent assay; and the contents of synovial MIF were detected by Western blot. Hematoxylin-eosin (HE) staining method was used to observe the pathological changes of synovial tissue under a section light microscope, and pathological scoring was performed according to the grading standard of the degree of synovial tissue disease.

RESULTS

Moxibustion was found to reduce the level of MIF and alleviate inflammation in RA rats in this study. In addition, after inhibiting the expression of MIF, the level of CORT increased, and the level of TNF-α decreased. Treating RA rats with inhibited MIF by moxibustion, the level of CORT was almost unchanged, but the level of TNF-α further decreased. The correlation analysis data suggested that MIF was positively related to the expression of TNF-α and negatively correlated with the expression of CORT.

CONCLUSION

Reducing MIF to increase CORT and decrease TNF-α by moxibustion treatment in RA. MIF may be a factor for moxibustion to regulate the expression of CORT, but the expression of TNF-α is due to the incomplete regulation of the MIF. This study added to the body of evidence pointing to moxibustion's anti-inflammatory mechanism in the treatment of RA.

目的

通过调节巨噬细胞移动抑制因子(MIF)/糖皮质激素(GCs)的表达,验证艾灸可能抑制类风湿关节炎(RA)滑膜炎症的假说。

方法

将 50 只雄性 Sprague-Dawley 大鼠随机分为 5 组(每组 10 只):空白对照组(CON)、RA 模型组(RA)、艾灸组(MOX)、MIF 抑制剂(S,R)-3-(4-羟苯基)-4,5-二氢-5-异恶唑乙酸甲酯(ISO-1)组、艾灸+MIF 抑制剂 ISO-1(MOX+ISO-1)组。ISO-1 组和 ISO-1+MOX 组大鼠腹腔内注射抑制剂 ISO-1。RA 组、ISO-1 组、MOX 组和 ISO-1+MOX 组大鼠右后足底注射完全弗氏佐剂(FCA)建立实验性 RA 大鼠模型。MOX 组和 MOX+ISO-1 组大鼠给予艾灸治疗。分别于造模前、造模后、艾灸治疗后 3 个时间点测量各组大鼠足底厚度。酶联免疫吸附试验(ELISA)检测血清 MIF、皮质酮(CORT)、肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)含量;Western blot 法检测滑膜 MIF 含量。苏木精-伊红(HE)染色法观察关节组织切片的病理变化,根据滑膜组织疾病程度分级标准进行病理评分。

结果

本研究发现艾灸可降低 RA 大鼠 MIF 水平并缓解炎症。此外,抑制 MIF 表达后,CORT 水平升高,TNF-α 水平降低。用艾灸抑制 MIF 治疗 RA 大鼠,CORT 水平几乎不变,但 TNF-α 进一步降低。相关分析数据表明,MIF 与 TNF-α 的表达呈正相关,与 CORT 的表达呈负相关。

结论

艾灸降低 MIF 可增加 CORT、降低 TNF-α。艾灸可能通过降低 MIF 来调节 CORT 的表达,但 TNF-α 的表达是由于 MIF 不完全调节所致。本研究为艾灸治疗 RA 的抗炎机制提供了更多证据。
