Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease, which will eventually lead to joints deformity and functional damage. The aim of this research is to evaluate the effect of moxibustion on the serum indicators related to bone and cartilage metabolism, matrix metalloproteinase 1 (MMP-1), matrix metalloproteinase 3 (MMP-3), and vascular endothelial growth factor (VEGF) in patients with RA and to explore the mechanism of moxibustion in the treatment of RA.
We recruited 70 RA patients who met the inclusion criteria, and they were randomly divided into two groups, a treatment group and a control group in equal ratio. The control group took methotrexate, folate, or leflunomide orally, while the treatment group received methotrexate, folate, or leflunomide orally and moxibustion at ST36 (Zusanli), BL23 (Shen shu), and Ashi points. We compared the clinical symptoms, RA serological disease markers and serum contents of interleukin-1 (IL-1), tumor necrosis factor- (TNF-), MMP-1, MMP-3, and VEGF of RA patients before and after treatment.
(1) The clinical symptoms and RA serological disease markers of the two groups improved after treatment ( < 0.05), while the clinical symptoms of the treatment group were significantly improved in comparison with the control group ( < 0.05). (2) The levels of IL-1, TNF- and VEGF decreased in both groups after treatment ( < 0.05), but the treatment group was significantly decreased compared with the control group ( < 0.05). (3) There were significant differences in MMP-1 and MMP-3 contents after treatment in the treatment group ( < 0.05, < 0.05), while there were no significant differences in the control group ( > 0.05, > 0.05). Above all, the contents of IL-1, TNF-, MMP-1, MMP-3, and VEGF in the treatment group decreased more significantly than those in the control group ( < 0.05).
The improvement effect of moxibustion on the clinical symptoms of RA patients may be related to influence on the contents of IL-1, TNF-, MMP-1, MMP-3, and VEGF, and moxibustion may play a potential role in bone protection.
类风湿关节炎(RA)是一种慢性炎症性自身免疫性疾病,最终会导致关节畸形和功能损害。本研究旨在评估艾灸对RA患者血清中与骨和软骨代谢相关指标、基质金属蛋白酶1(MMP - 1)、基质金属蛋白酶3(MMP - 3)及血管内皮生长因子(VEGF)的影响,并探讨艾灸治疗RA的作用机制。
招募70例符合纳入标准的RA患者,将其随机等分为治疗组和对照组。对照组口服甲氨蝶呤、叶酸或来氟米特,治疗组在口服甲氨蝶呤、叶酸或来氟米特的基础上加用艾灸足三里(ST36)、肾俞(BL23)及阿是穴。比较两组RA患者治疗前后的临床症状、RA血清学疾病标志物以及白细胞介素 - 1(IL - 1)、肿瘤坏死因子 - (TNF - )、MMP - 1、MMP - 3和VEGF的血清含量。
(1)两组治疗后临床症状及RA血清学疾病标志物均改善(P < 0.05),但治疗组临床症状改善程度明显优于对照组(P < 0.05)。(2)两组治疗后IL - 1、TNF - 和VEGF水平均降低(P < 0.05),但治疗组降低幅度明显大于对照组(P < 0.05)。(3)治疗组治疗后MMP - 1和MMP - 3含量有显著差异(P < 0.05,P < 0.05),而对照组无显著差异(P > 0.05,P > 0.05)。综上所述,治疗组IL - 1、TNF - 、MMP - 1、MMP - 3和VEGF含量下降幅度明显大于对照组(P < 0.05)。
艾灸对RA患者临床症状的改善作用可能与影响IL - 1、TNF - 、MMP - 1、MMP - 3和VEGF含量有关,艾灸可能在骨保护方面发挥潜在作用。
