Rectus sheath hematoma (RSH) is uncommon, and because people have limited knowledge about it, it is difficult to recognize the symptoms in time, often de-laying optimal treatment.
Herein, we report a case of a 77-year-old female with RSH. The patient was treated at our hospital for coronavirus disease 2019. Anticoagulant treatment was administered during this period because of thrombosis. On the 8 d of treatment, the patient complained of abdominal pain. Ultrasonography revealed a solid cystic mass in the pelvic cavity. An emergency laparotomy was performed, and a huge hematoma was found in the deep layer of the rectus abdominis muscle. We used anticoagulants with caution based on the patient's condition.
Optimal management of patients with RSH s depends on timely diagnosis and when to reintroduce anticoagulants.
腹直肌鞘血肿（RSH）并不常见，由于人们对此了解有限，难以及时识别症状，常常延误最佳治疗时机。
在此，我们报告一例77岁女性腹直肌鞘血肿病例。该患者因2019冠状病毒病在我院接受治疗。在此期间，因血栓形成给予了抗凝治疗。治疗第8天，患者诉腹痛。超声检查显示盆腔有一实性囊性肿块。遂行急诊剖腹探查术，发现腹直肌深层有一巨大血肿。我们根据患者情况谨慎使用抗凝剂。
腹直肌鞘血肿患者的最佳治疗取决于及时诊断以及何时重新使用抗凝剂。
