Abstract

OBJECTIVE

To explore the application effect of ward noise management during the perioperative period of hepatobiliary surgery.

METHODS

The clinical data of 295 patients undergoing hepatobiliary surgery admitted to People's Hospital of Zunyi City Bo Zhou District from March 2020 to March 2023 were retrospectively analyzed. In accordance with different perioperative management programs, patients were divided into the control (implementation of perioperative routine management) and observation (implementation of perioperative routine management + ward noise management) groups. Patients' general data were matched through propensity score matching, and 55 cases were allocated to each group. After matching, the clinical indicators of the two groups were compared to evaluate the effect of ward noise management on patients undergoing hepatobiliary surgery.

RESULTS

No significant difference in general data was found between the two groups (P > 0.05). After management, the postoperative recovery indicators, such as feeding time, exhaust time, defecation time, first time to get out of bed, and incidence of postoperative complications, did not significantly differ between the observation and control groups (P > 0.05). The Hamilton Anxiety Scale, Hamilton Depression Scale and Pittsburgh Sleep Quality Index scores of the observation group were lower than those of the control group (P < 0.05). The average noise decibel values during the day, night, and over 24 hours of the observation group were lower than those of the control group (P < 0.05).

CONCLUSIONS

Ward noise management can improve the negative emotions of patients undergoing hepatobiliary surgery, enhance sleep quality, and promote recovery. Therefore, it has a certain clinical promotion value.

目的

探讨肝胆外科围手术期病房噪声管理的应用效果。

方法

回顾性分析 2020 年 3 月至 2023 年 3 月遵义市播州区人民医院收治的 295 例肝胆外科手术患者的临床资料。根据围手术期管理方案的不同,将患者分为对照组(实施围手术期常规管理)和观察组(实施围手术期常规管理+病房噪声管理),采用倾向性评分匹配法对患者的一般资料进行匹配,每组 55 例。匹配后比较两组患者的临床指标,评价病房噪声管理对肝胆外科手术患者的影响。

结果

两组患者的一般资料比较,差异无统计学意义(P>0.05)。管理后,观察组患者的术后恢复指标,如进食时间、排气时间、排便时间、首次下床时间和术后并发症发生率,与对照组比较,差异均无统计学意义(P>0.05)。观察组患者的汉密尔顿焦虑量表、汉密尔顿抑郁量表和匹兹堡睡眠质量指数评分均低于对照组(P<0.05)。观察组患者白天、夜间和 24 小时的平均噪声分贝值均低于对照组(P<0.05)。

结论

病房噪声管理可改善肝胆外科手术患者的负性情绪,提高睡眠质量,促进康复,具有一定的临床推广价值。
