音乐疗法对新生儿重症监护病房早产儿的临床效果观察。

Observations on the Clinical Effects of Music Therapy on Premature Infants in Neonatal Intensive Care Units.

Abstract

OBJECTIVE

This study aimed to explore the clinical effects of music therapy (MT) on premature infants in neonatal intensive care units (NICUs).

METHODS

A total of 78 premature infants in NICUs admitted from January 2021 to January 2022 in Wuhan Children's Hospital, Tongji Medical College, and Huazhong University of Science and Technology were selected as the reference group and received routine management. Wuhan Children's Hospital implemented MT from February 2022 to February 2023, and 74 premature infants in NICUs admitted during the same period were selected as the observation group. The corresponding management mode was implemented on the second day of NICU admission for premature infants. Amplitude-integrated electroencephalogram (aEEG) and neonatal behavioral neurological assessment (NBNA) were adopted to evaluate the brain function, specifically the neurological function of neonates, after the end of management. The degree of parent-child attachment was measured using the pictorial representation of attachment measure (PRAM) in a nonverbal (visual) manner. The temperature, pulse, respiratory rate (RR), the number and duration of crying, and incidence of complications were all compared between the two groups.

RESULTS

SPSS showed that no difference existed in the aEEG and NBNA scores between the two groups (P > 0.05). The distance of PRAM self-baby-distance was smaller in the observation group compared with the reference group (P < 0.05). Furthermore, there was no significant difference in temperature between the two groups (P > 0.05). The observation group had significantly lower pulse and RR values than the reference group (P < 0.05). Finally, no significant difference existed in the incidence of complications between the two groups (P > 0.05).

CONCLUSION

MT has a certain application value for premature infants in NICUs and can thus be applied to newborns in other NICUs. However, further studies are required to completely verify the research results.

目的

本研究旨在探讨音乐疗法(MT)对新生儿重症监护病房(NICU)早产儿的临床效果。

方法

选取 2021 年 1 月至 2022 年 1 月华中科技大学同济医学院附属武汉儿童医院收治的 78 例 NICU 早产儿为对照组,给予常规管理。自 2022 年 2 月起,华中科技大学同济医学院附属武汉儿童医院开始实施音乐疗法,选取同期收治的 74 例 NICU 早产儿为观察组。两组患儿均于 NICU 入院次日开始实施相应管理模式。采用振幅整合脑电图(aEEG)和新生儿行为神经评估(NBNA)评估管理结束后早产儿的脑功能,即新生儿的神经功能。采用非言语(视觉)方式的图画依恋测量(PRAM)评估两组患儿的亲子依恋程度。比较两组患儿的体温、脉搏、呼吸频率(RR)、哭闹次数和时间、并发症发生率。

结果

SPSS 结果显示,两组患儿的 aEEG 和 NBNA 评分比较,差异无统计学意义(P>0.05)。观察组患儿 PRAM 自婴儿距离得分小于对照组,差异有统计学意义(P<0.05)。两组患儿的体温比较,差异无统计学意义(P>0.05)。观察组患儿的脉搏和 RR 均低于对照组,差异有统计学意义(P<0.05)。两组患儿的并发症发生率比较,差异无统计学意义(P>0.05)。

结论

音乐疗法对 NICU 早产儿具有一定的应用价值,可应用于其他 NICU 的新生儿,但仍需进一步研究以完全验证研究结果。
