Abstract

OBJECTIVE

Comparing fixation procedures for managing naso-orbito-ethmoidal (NOE) fractures.

METHODS

Group A (plate and screw fixation), Group B (Kirschner wire (K-wire) fixation), Group C (absorbable plate and screw fixation), and Group D (No fixation) were formed from 120 NOE fracture patients. The facial injury severity scale, visual analogue scale, CT scans, and clinical examination examined functional and cosmetic outcomes, complications, and postoperative stability.

RESULTS

The study found that plate and screw fixation (Group A) had the highest stability and aesthetic results, followed by absorbable plate and screw fixation (Group C). Conservative management (Group D) had the worst outcomes, while K-wire fixation (Group B) had good results but more complications. Results showed substantial differences in functional, aesthetic, complications, and postoperative stability between groups ( < 0.05).

CONCLUSION

Plate and screw fixation is the most successful and dependable approach for treating NOE fractures, with superior functional and aesthetic outcomes, reduced complications, and higher postoperative stability than K-wire fixation, absorbable plate and screw fixation, and conservative care. Absorbable plate and screw fixation may function instead of metal. Conservative care should be rare. Larger multicenter randomized controlled trials should corroborate these findings.

目的

比较治疗鼻眶筛(NOE)骨折的固定方法。

方法

将120例NOE骨折患者分为A组(钢板螺钉固定)、B组(克氏针固定)、C组(可吸收钢板螺钉固定)和D组(不固定)。采用面部损伤严重程度量表、视觉模拟量表、CT扫描及临床检查来评估功能和美容效果、并发症及术后稳定性。

结果

研究发现,钢板螺钉固定(A组)稳定性和美学效果最佳,其次是可吸收钢板螺钉固定(C组)。保守治疗(D组)效果最差,而克氏针固定(B组)效果良好但并发症较多。结果显示各组在功能、美学、并发症及术后稳定性方面存在显著差异(<0.05)。

结论

钢板螺钉固定是治疗NOE骨折最成功、可靠的方法,与克氏针固定、可吸收钢板螺钉固定及保守治疗相比,具有更好的功能和美学效果,并发症更少,术后稳定性更高。可吸收钢板螺钉固定可替代金属固定。保守治疗应尽量少用。需要更大规模的多中心随机对照试验来证实这些发现。
