We report a rare case of a 53-year-old woman presenting with pulmonary thromboembolic disease and a right atrial mass initially suspected to be myxoma. Imaging and surgical exploration revealed the mass to be accessory hepatic tissue extending into the inferior vena cava, a highly unusual finding. Histopathologic examination confirmed the diagnosis. This case underscores the importance of considering rare entities like accessory hepatic tissue in the differential diagnosis of right atrial masses to guide appropriate surgical management and prevent complications.
