To compare the clinical effects of using xenogenic bone graft materials with or without collagen components for tooth micro crestal flap-alveolar ridge preservation (MCF-ARP) at molars with severe periodontitis. This study included patients who visited Department of Periodontology, Peking University School and Hospital of Stomatology from May to November 2023 due to severe periodontitis, requiring tooth extraction and planning for implant-retained prostheses. A total of 24 molars from 24 patients with severe periodontitis were assigned into two groups: the deproteinized bovine bone mineral (DBBM) group and the DBBM with collagen (DBBM-C) group. Twelve affected teeth from 12 patients were included in each group. Both groups underwent minimally invasive tooth extraction and MCF-ARP, with DBBM and DBBM-C implanted in the extraction socket, respectively. Cone beam CT (CBCT) was performed before and 6 months after the surgery for assessing changes of hard tissue. Intraoral scanning was performed before and at 2 weeks, 1 month, 3 months, and 6 months after the surgery for assessing soft tissue contour changes and patterns in both groups. After 6 months of healing, the central bone height increased by (8.35±2.25) mm in the DBBM group and (7.70±2.36) mm in the DBBM-C group. The ridge width at 1 mm apically from the higher bone crest increased by 6.43 (-0.76,7.96) mm in the DBBM group and 6.01 (5.41,7.90) mm in the DBBM-C group. There was no statistically significant difference in the changes of bone height and width between the two groups (all >0.05). In terms of soft tissue contour changes, although the buccal contour collapses were less in the DBBM-C group, the two groups showed no statistically significant difference (all >0.05). Within the limitations of this study, it was demonstrated that the clinical effects of MCF-ARP using xenogenic bone graft materials with or without collagen components in molars with severe periodontitis were comparable.
比较使用含或不含胶原蛋白成分的异种骨移植材料对重度牙周炎磨牙进行牙微嵴顶瓣-牙槽嵴保存术(MCF-ARP)的临床效果。本研究纳入了2023年5月至11月因重度牙周炎就诊于北京大学口腔医学院牙周科、需要拔牙并计划进行种植修复的患者。将24例重度牙周炎患者的24颗磨牙分为两组:脱蛋白牛骨矿物质(DBBM)组和含胶原蛋白的DBBM(DBBM-C)组。每组纳入12例患者的12颗患牙。两组均接受微创拔牙和MCF-ARP,分别在拔牙窝植入DBBM和DBBM-C。术前及术后6个月进行锥形束CT(CBCT)检查以评估硬组织变化。术前及术后2周、1个月、3个月和6个月进行口内扫描,以评估两组软组织轮廓变化和形态。愈合6个月后,DBBM组中央骨高度增加(8.35±2.25)mm,DBBM-C组增加(7.70±2.36)mm。距较高骨嵴顶1 mm处的牙槽嵴宽度,DBBM组增加6.43(-0.76,7.96)mm,DBBM-C组增加6.01(5.41,7.90)mm。两组骨高度和宽度变化差异无统计学意义(均>0.05)。在软组织轮廓变化方面,虽然DBBM-C组颊侧轮廓塌陷较少,但两组差异无统计学意义(均>0.05)。在本研究的局限性内,结果表明,在重度牙周炎磨牙中使用含或不含胶原蛋白成分的异种骨移植材料进行MCF-ARP的临床效果相当。
