Scrotal trauma, though uncommon, can rarely result in isolated epididymal injury, as seen in a rare case of bilateral epididymal injury leading to obstructive infertility in a 23-year-old male who sustained blunt scrotal trauma. Doppler ultrasound revealed bilateral hematocele and epididymal discontinuity, which was confirmed by surgical exploration showing bilateral epididymal lacerations. Postoperative semen analysis indicated azoospermia. Isolated epididymal injuries are rarely diagnosed early, emphasizing the importance of thorough imaging and consideration of fertility preservation. Urologists should consider early sperm preservation in cases with a high risk of infertility.
阴囊创伤虽不常见,但极少会导致单纯性附睾损伤,如在一例罕见的双侧附睾损伤病例中所见,该病例为一名23岁男性因阴囊钝性创伤导致梗阻性不育。多普勒超声显示双侧阴囊积血和附睾连续性中断,手术探查证实为双侧附睾撕裂伤。术后精液分析显示无精子症。单纯性附睾损伤很少能早期诊断,这凸显了全面影像学检查和考虑生育力保留的重要性。对于存在不育高风险的病例,泌尿外科医生应考虑早期进行精子保存。
