Bilateral retinal artery occlusion (RAO) in patients with Type 2 diabetes mellitus is an extremely rare condition. Even with rigorous treatment, RAO often results in devastating visual impairment.
A 50-year-old female with Type 2 diabetes mellitus was referred to our hospital. Notably, she developed bilateral RAO sequentially within a mere five-month period. This rapid progression of the disease in both eyes is of particular interest.
Fundus examination confirmed branch retinal artery occlusion (BRAO) in her left eye and central retinal artery occlusion (CRAO) in her right eye. After timely intra-arterial thrombolysis, significant bilateral visual recovery was achieved. The patient's visual acuity in the left eye improved from 20/400 to 20/100 and in the right eye from 20/2000 to 20/33.3, indicating the effectiveness of this treatment approach in this case. Further examinations also showed positive results: the cherry-red spot in the macula disappeared, FFA indicated improved retinal artery perfusion, and OCT revealed a certain degree of restoration in the retinal structure.
This case highlights that metabolic disorders, like hyperlipidemia and diabetes, can be considered high-risk factors for the development of RAO. Prompt and effective intervention, such as timely intra-arterial thrombolysis, as demonstrated in this case, is crucial for preserving patients' visual prognosis.
2型糖尿病患者双侧视网膜动脉阻塞(RAO)是一种极其罕见的病症。即使进行严格治疗,RAO通常也会导致严重的视力损害。
一名50岁的2型糖尿病女性被转诊至我院。值得注意的是,她在短短五个月内先后出现双侧RAO。双眼疾病的这种快速进展尤其令人关注。
眼底检查证实其左眼为视网膜分支动脉阻塞(BRAO),右眼为视网膜中央动脉阻塞(CRAO)。经过及时的动脉内溶栓治疗,双眼视力显著恢复。患者左眼视力从20/400提高到20/100,右眼视力从20/2000提高到20/33.3,表明该治疗方法在该病例中有效。进一步检查也显示出阳性结果:黄斑区樱桃红斑消失,荧光素血管造影(FFA)显示视网膜动脉灌注改善,光学相干断层扫描(OCT)显示视网膜结构有一定程度的恢复。
该病例突出表明,高脂血症和糖尿病等代谢紊乱可被视为RAO发生的高危因素。如本病例所示,及时有效的干预措施,如及时的动脉内溶栓治疗,对于保留患者的视力预后至关重要。
