Abstract

PURPOSE

Compared the impact of ultrasound contrast-enhanced fusion virtual navigation technology-guided microwave ablation (MWA) and standard MWA on the local tumor progression of hepatocellular carcinoma with unclear ultrasound images, to explore the utility of virtual navigation ultrasound contrast-enhanced imaging.

METHODS

A retrospective analysis was conducted on clinical data from hepatocellular carcinoma patients admitted between June 2021 and August 2022, all of whom had liver lesions suggested by Computed Tomography (CT) or Magnetic Resonance (MR) but not visualized on 2D ultrasound. They underwent ultrasound contrast-enhanced fusion virtual navigation technology-guided microwave ablation and standard microwave ablation, and comparisons were made between the two groups in terms of general information, blood parameters, ultrasonic test, intraoperative indicators, ablation indicators, postoperative complications, recurrence indicators, local progression and long-term outcomes.

RESULTS

There were no significant differences in blood parameters, postoperative complications, and disease-free survival between the two groups. (P > 0.05). The ultrasound contrast-enhanced fusion virtual navigation technology-guided microwave ablation group had significantly higher lesion detection rate, ablation efficacy, proportion of safe margin > 5 mm, recurrence time and complete ablation rate compared to the standard microwave ablation group, with significantly lower ablation time, recurrence rate and residual disease at 1-month compared to conventional ablation and significant differences in local progression (P < 0.05).

CONCLUSION

Ultrasound contrast-enhanced fusion virtual navigation, as a novel ultrasound contrast-enhanced technique, enabled the visualization of liver lesions that are indistinct on 2D ultrasound imaging, significantly enhancing lesion detection and microwave ablation efficiency. It also reduced local progression and recurrence rates, demonstrating significant clinical utility. Its advantages positioned it as a critical tool for precise ablation localization and treatment.

目的

比较超声造影增强融合虚拟导航技术引导下的微波消融(MWA)与标准MWA对超声图像不清晰的肝细胞癌局部肿瘤进展的影响,探讨虚拟导航超声造影成像的应用价值。

方法

对2021年6月至2022年8月收治的肝细胞癌患者的临床资料进行回顾性分析,所有患者均经计算机断层扫描(CT)或磁共振成像(MR)提示有肝脏病变,但二维超声未显示。他们接受了超声造影增强融合虚拟导航技术引导下的微波消融和标准微波消融,并对两组患者的一般资料、血液参数、超声检查、术中指标、消融指标、术后并发症、复发指标、局部进展情况和长期预后进行了比较。

结果

两组患者的血液参数、术后并发症和无病生存率无显著差异(P>0.05)。与标准微波消融组相比,超声造影增强融合虚拟导航技术引导下的微波消融组的病灶检出率、消融效果、安全边缘>5mm的比例、复发时间和完全消融率显著更高,与传统消融相比,消融时间、复发率和1个月时的残留病灶显著更低,局部进展情况有显著差异(P<0.05)。

结论

超声造影增强融合虚拟导航作为一种新型的超声造影技术,能够使二维超声成像中不清晰的肝脏病灶可视化,显著提高病灶检出率和微波消融效率。它还降低了局部进展和复发率,并显示出显著的临床应用价值。其优势使其成为精确消融定位和治疗的关键工具。
