We describe a case of a female in her 20s presenting with recurrent, progressive, asymmetrical, globular swelling over the posterior aspect of both lower limbs and ankles. The patient had previously undergone bilateral tendoachilles tumour excision elsewhere. A thorough history, examination and previous reports confirmed the recurrence of tendoachilles xanthoma. Blood investigations revealed hypercholesterolaemia. Surgical intervention, involving tumour excision and flexor digitorum longus transfer, was performed in two stages for each leg. Subsequent follow-up and subjective evaluation demonstrated satisfactory outcomes.
