INTRODUCTION
This study aimed to investigate the knowledge, attitudes, and practices (KAP) of patients with lumbar disc herniation (LDH) regarding their condition.
METHODS
A cross-sectional study was conducted between June and September 2024 at Tongren Hospital in Shanghai and the Seventh People's Hospital of Changzhou, targeting patients diagnosed with LDH. Demographic information and KAP scores were collected and evaluated through a structured questionnaire. The Oswestry Disability Index (ODI) was used to assess the level of disability experienced by patients in daily activities.
RESULTS
A total of 395 valid questionnaires were collected, yielding an effective rate of 84.40%. Of the respondents, 205 (51.90%) were female, 201 (50.89%) had undergone surgical treatment. The mean ± SD scores for knowledge, attitude, practice, and ODI were 14.91 ± 5.07 (possible range: 0-22), 39.26 ± 3.78 (possible range: 10-50), 39.48 ± 6.70 (possible range: 11-55), and 12.92 ± 9.60 (possible range: 0-50), respectively. The Structural Equation Model indicated that knowledge had a direct effect on both attitude ( = 0.458, = 0.006) and practice ( = 0.214, = 0.002), while attitude directly affected practice ( = 0.323, = 0.008). Knowledge also indirectly influenced practice via attitude ( = 0.148, = 0.006).
CONCLUSION
Patients with LDH demonstrated inadequate knowledge but generally positive attitudes and proactive practices regarding their condition, along with a moderate level of disability related to low back pain. Improving patient education, particularly focusing on enhancing knowledge, may foster more positive attitudes and better self-management practices, potentially reducing disability.

本研究旨在调查腰椎间盘突出症(LDH)患者对自身病情的认知、态度和行为(KAP)。
方法
2024年6月至9月期间,在上海同仁医院和常州第七人民医院开展了一项横断面研究,目标对象为被诊断为腰椎间盘突出症的患者。通过结构化问卷收集并评估人口统计学信息和KAP得分。采用Oswestry功能障碍指数(ODI)评估患者在日常活动中所经历的功能障碍程度。
结果
共收集到395份有效问卷,有效率为84.40%。在受访者中,女性有205人(51.90%),接受过手术治疗的有201人(50.89%)。知识、态度、行为和ODI的平均±标准差得分分别为14.91±5.07(可能范围:0 - 22)、39.26±3.78(可能范围:10 - 50)、39.48±6.70(可能范围:11 - 55)和12.92±9.60(可能范围:0 - 50)。结构方程模型表明,知识对态度(β = 0.458,P = 0.006)和行为(β = 0.214,P = 0.002)均有直接影响,而态度直接影响行为(β = 0.323,P = 0.008)。知识还通过态度间接影响行为(β = 0.148,P = 0.006)。
结论
腰椎间盘突出症患者对自身病情的知识掌握不足,但态度总体积极,行为较为主动,同时与腰痛相关的功能障碍程度中等。改善患者教育,特别是注重增强知识,可能会培养更积极的态度和更好的自我管理行为,从而有可能减轻功能障碍。
