Cardiac rehabilitation is a widely adopted approach for postoperative recovery in coronary heart disease patients undergoing coronary artery bypass grafting (CABG). However, in China, patients tend to be highly dependent on medical staff and less expressive of their personal wishes in medical decision-making due to the influence of traditional concepts. This study aims to investigate the current status of CABG postoperative patients' perceptions of shared decision-making during cardiac rehabilitation.
Using the Colaizzi's phenomenological method, 19 post-CABG from three tertiary general hospital in Shanghai were purposively selected for semi-structured in-depth interviews between February 2025 to April 2025.
Two main themes and eight subthemes emerged: (1) Patients' perceptions of SDM participation during cardiac rehabilitation, including insufficient understanding of the role, restricted decision-making capacity, impact of medical staff-patient communication quality, and psychological barriers to engagement; (2) Patients' needs for shared decision-making participation during cardiac rehabilitation, knowledge requirements for cardiac rehabilitation, continuity of medical resources, family support, and decision aids.
Post-CABG patients exhibit limited awareness of shared decision-making and face multifaceted barriers to participation. Future interventions should enhance patients' decision-making awareness and address their needs through education, resource continuity, family engagement, and tailored decision aids to improve rehabilitation adherence and quality of life.
心脏康复是冠状动脉旁路移植术(CABG)后冠心病患者术后恢复广泛采用的方法。然而,在中国,由于传统观念的影响,患者往往高度依赖医务人员,在医疗决策中较少表达个人意愿。本研究旨在调查CABG术后患者在心脏康复期间对共同决策的认知现状。
采用Colaizzi现象学方法,于2025年2月至2025年4月从上海三家三级综合医院中选取19例CABG术后患者进行半结构化深入访谈。
出现了两个主要主题和八个子主题:(1)患者对心脏康复期间共同决策参与的认知,包括对角色理解不足、决策能力受限、医护沟通质量的影响以及参与的心理障碍;(2)患者对心脏康复期间共同决策参与的需求,心脏康复知识要求、医疗资源连续性、家庭支持和决策辅助工具。
CABG术后患者对共同决策的认识有限,参与面临多方面障碍。未来的干预措施应通过教育、资源连续性、家庭参与和量身定制的决策辅助工具来提高患者的决策意识并满足其需求,以改善康复依从性和生活质量。
