Vascular compression by osteocartilaginous structures, such as the hyoid bone, is an underrecognized cause of ischemic stroke, often referred to as "bony stroke." Due to its rarity, this condition presents a significant diagnostic challenge, requiring meticulous imaging to identify the underlying structural abnormalities. We report an intriguing case of a 40-year-old female presenting with an acute embolic ischemic stroke. Diagnostic imaging revealed compression of the right internal carotid artery by the greater horn of the hyoid bone, causing arterial stagnation in both neutral and rotated neck positions, which resolved after surgical decompression. Through this case, we emphasize the importance of a comprehensive evaluation of osteocartilaginous cervical structures surrounding the brain-supplying arteries, as they represent a rare yet treatable cause of ischemic stroke.
骨软骨结构(如舌骨)对血管的压迫是缺血性中风的一个未被充分认识的原因,常被称为“骨性中风”。由于其罕见性,这种情况带来了重大的诊断挑战,需要细致的影像学检查来识别潜在的结构异常。我们报告了一例有趣的病例,一名40岁女性出现急性栓塞性缺血性中风。诊断性影像学检查显示舌骨大角压迫右侧颈内动脉,在颈部中立位和旋转位均导致动脉血流停滞,手术减压后症状缓解。通过这个病例,我们强调了对供应大脑的动脉周围的骨软骨性颈椎结构进行全面评估的重要性,因为它们是缺血性中风的一种罕见但可治疗的原因。
