Abstract

PURPOSE

To investigate ultrawidefield optical coherence tomography angiography (UWF-OCTA) changes in the mid-periphery of new-onset active Vogt-Koyanagi-Harada (VKH) patients.

METHODS

New-onset and untreated active VKH patients were continuously observed using UWF-OCTA at 2 weeks, 1, and 3 months after treatment.

RESULTS

Twenty-four new-onset and untreated active VKH patients were included. Seven inactive VKH patients with sunset glow fundus (SGF), and 10 age-sex-matched normal people served as controls. In active VKH patients, there was a significant change in choroidal flow but no change in retinal flow in the mid-periphery before treatment. Vascular density of large- and medium-sized choroidal vessels (LMCV-VD) in mid-periphery, along with retinal and choroidal capillaries in macula, was decreased. Retinal and choroidal thickness in both macula and mid-periphery were increased (all  < 0.05). After 2 weeks of treatment, the parameters of the retina and choroid in both the mid-periphery and the macula were significantly improved. Retinal thickness and LMCV-VD recovered sequentially: first in the macular and temporal regions, followed by the nasal area. Deep capillary plexus-VD in the macula at 3 months of treatment was correlated with visual acuity ( = 0.699,  = 0.002). Binary logistic regression ( = 0.044, OR = 2.695, 95% CI95% CI = 1.027 ~ 7.056) analyses revealed that LMCV-VD at 2 weeks of treatment was associated with the prediction of SGF.

CONCLUSION

In active VKH patients, the parameters of the retina and choroid were altered in both mid-periphery and macula. After treatment, the temporal side was prioritized for recovery. LMCV-VD at two weeks of treatment had the potential to predict the SGF.

目的

研究初发性活动性Vogt-小柳-原田病(VKH)患者中周部的超广角光学相干断层扫描血管造影(UWF-OCTA)变化。

方法

对初发性且未经治疗的活动性VKH患者在治疗后2周、1个月和3个月时使用UWF-OCTA进行连续观察。

结果

纳入24例初发性且未经治疗的活动性VKH患者。7例有晚霞样眼底(SGF)的非活动性VKH患者和10例年龄及性别匹配的正常人为对照组。在活动性VKH患者中,治疗前中周部脉络膜血流有显著变化,但视网膜血流无变化。中周部大、中型脉络膜血管(LMCV-VD)以及黄斑区视网膜和脉络膜毛细血管的血管密度降低。黄斑区和中周部的视网膜和脉络膜厚度均增加(均P<0.05)。治疗2周后,中周部和黄斑区的视网膜和脉络膜参数均显著改善。视网膜厚度和LMCV-VD依次恢复:首先是黄斑区和颞侧区域,随后是鼻侧区域。治疗3个月时黄斑区深层毛细血管丛-VD与视力相关(r = 0.699,P = 0.002)。二元逻辑回归分析(P = 0.044,OR = 2.695,95%CI = 1.027~7.056)显示,治疗2周时的LMCV-VD与SGF的预测相关。

结论
