A rare case of spontaneous intrahepatic cutaneous biliary fistula due to obstruction of the biliary tree by ampullary fibrosis is reported. The diagnosis was made clinically by the presence of constant flow of the bile through the fistulous opening and confirmed by roentgenography and by surgery. The definitive treatment consisted of removal of the obstruction by choledochoduodenostomy. The fistula healed spontaneously within two weeks of surgery.
