•Initial structural remodeling, indexed by decreases in left ventricular end-diastolic volume, induced by the Transcatheter Left Ventricular Restoration System is followed by a period of biological reverse remodeling that plateaus after about 6 months and is maintained at least through 2 years.•These structural changes are associated with ∼5% (absolute) increases in left ventricular ejection fraction.•These changes occur in the backdrop of relatively few changes in heart failure medical therapy.•These changes are also associated with improvement of quality of life, though this observation needs to be considered in the context of the current unblinded study.
