Adenoid cystic carcinoma of the breast (ACCB) is a rare and distinct form of triple-negative breast cancer, accounting for only 0.06-0.1% of all breast cancers. ACCB is characterized by a favorable prognosis and can be divided into three subtypes: Classical, solid and high-grade transformation. The classical type is the most common and has the least aggressive biological behavior, with rare occurrences of metastasis. By contrast, patients with the solid and high-grade transformation types are more likely to experience local recurrence and distant metastasis. The present report describes a rare case of hepatic metastasis in classical ACCB, and discusses its treatment and follow-up. Additionally, the current advancements in the management of ACCB are reviewed to provide insights for further understanding and clinical guidance.
