In recent years, research on the relationship between hepatitis B virus-related cirrhosis (HBV-LC) and gut microbiota has grown, but studies focusing on the Northwest Chinese population are scarce. This study characterized the gut microbiota composition and function in HBV-LC patients vs. healthy individuals in Northwest China, aiming to provide a scientific basis for region-specific precision therapies.
A cross-sectional study enrolled 43 HBV-LC patients and 43 age-/sex-matched healthy controls (HC) from Gansu Province. Clinical parameters including liver function, blood routine, coagulation function, blood biochemistry were measured. Shotgun metagenomic sequencing was conducted to analyze gut microbiota taxonomic composition and function.
HBV-LC patients showed significantly elevated alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), γ-glutamyl transferase (γ-GGT), prothrombin time, international normalized ratio (INR), and thrombin time, but reduced triglycerides (TG), total cholesterol (TC), erythrocytes, thrombocytes, total protein, albumin, and prothrombin time activity (PT-ratio). Alpha-diversity based on Shannon and Simpson indices was lower in HBV-LC. At the genus level, , , , , , and were enriched in HBV-LC, while , , , , , , and were reduced. Species-level analysis identified distinct enrichment of spp. in HBV-LC. Functional analysis revealed 482 metabolic pathways. HBV-LC showed enhanced lipid, amino acid, and nucleotide metabolism, menaquinol biosynthesis, and anaerobic energy metabolism, but reduced acetate/lactate production, lactose/galactose degradation, and peptidoglycan biosynthesis. Metagenome-wide association study revealed HBV-LC-enriched opportunistic species (e.g., spp.) correlated positively with hepatic enzymes and coagulation parameters, and negatively with TC, TG, and erythrocyte counts.
HBV-LC patients in Northwest China exhibit altered clinical indicators, gut microbial composition (reduced diversity, increased opportunistic pathogens, decreased beneficial species), and metabolic function. These findings highlight the potential of gut microbiome-targeted interventions for regional precision medicine of HBV-LC.

HBV-LC患者的丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、γ-谷氨酰转移酶(γ-GGT)、凝血酶原时间、国际标准化比值(INR)和凝血酶时间显著升高,但甘油三酯(TG)、总胆固醇(TC)、红细胞、血小板、总蛋白、白蛋白和凝血酶原时间活性(PT比值)降低。基于香农和辛普森指数的α多样性在HBV-LC中较低。在属水平上,[具体属名1]、[具体属名2]、[具体属名3]、[具体属名4]、[具体属名5]、[具体属名6]和[具体属名7]在HBV-LC中富集,而[具体属名8]、[具体属名9]、[具体属名10]、[具体属名11]、[具体属名12]、[具体属名13]和[具体属名14]减少。种水平分析确定了[具体种名]在HBV-LC中明显富集。功能分析揭示了482条代谢途径。HBV-LC显示脂质、氨基酸和核苷酸代谢、甲萘醌生物合成和无氧能量代谢增强,但乙酸盐/乳酸盐产生、乳糖/半乳糖降解和肽聚糖生物合成减少。全基因组关联研究表明,HBV-LC中富集的机会性物种(如[具体种名])与肝酶和凝血参数呈正相关,与TC、TG和红细胞计数呈负相关。
中国西北地区的HBV-LC患者表现出临床指标、肠道微生物组成(多样性降低、机会性病原体增加、有益物种减少)和代谢功能的改变。这些发现突出了针对肠道微生物群的干预措施在HBV-LC区域精准医学中的潜力。
