Patients with a history of anaphylactoid reaction to radiographic contrast medium (RCM) are at increased risk for subsequent reaction. To date, 275 of these high-risk patients have been studied with use of prophylactic pretreatment with diphenhydramine hydrochloride or prednisone or both without a single severe anaphylactoid reaction on readministration of RCM. This report describes a patient who received pretreatment as described in the literature but suffered a life-threatening anaphylactoid reaction on readministration of RCM. Premedication does not eliminate the possiblity of a fatal reaction on readministration of RCM.
对放射造影剂(RCM)有类过敏反应病史的患者,再次接触时发生反应的风险会增加。迄今为止,已对275例这类高危患者进行了研究,采用盐酸苯海拉明或泼尼松或两者联合进行预防性预处理,再次给予RCM时均未发生严重类过敏反应。本报告描述了一名患者,其接受了文献中所述的预处理,但再次给予RCM时发生了危及生命的类过敏反应。预处理并不能消除再次给予RCM时发生致命反应的可能性。
