Main elements of the diagnostics of sarcoidosis are clinical picure, thorax-roentgen and histologic findings, tuberculin- and kveim-test as well as behaviour of the findings under corticotherapy. Important for the diagnosis is the synopsis of several criteria. In the therapy are corticosteroids the remedy of choice. The judgment of the success of therapy is made difficult by a high number of cases with spontaneous healing and the intermittent course of sarcoidosis. While clinically relevant extra-thoracic manifestations always need a treatment, sarcoidosis in stage I and I-II, respectively, should receive corticosteroids only then, when the findings are progressive or when there is no tendency to regression during a period of observation of one year.
