While the pulmonary acinus as an anatomical unit is central to an understanding of pulmonary disease, its radiographic validity remains in doubt. A review of the historical basis of present knowledge about the acinus reveals several sources of confusion. It is suggested that recent findings provide better justification for the use of this term than historical sources.
