We present a patient with hypothyroidism secondary to Riedel's struma, treated with thyroxine and prednisolone, who developed clinical and radiological features of a unilateral orbital pseudotumour with lacrimal gland involvement; the histology of which was indistinguishable from that of the thyroid gland.
