Impalement injuries have to be differentiated corresponding to their position and extension. Concerning frequency the perineal region dominates. Surgery should be done as early as possible to control lesions of abdominal and/or thoracic organs and to avoid wound infection. At present the mortality rate came down to app. 10%.
