A case of monoamniotic twin pregnancy with a peculiar cord complication is presented. Embryologic single-ovum twinning is discussed. Attention is drawn to the fact that this clinical entity may not be as rare as we think. The hazards to the mother and the fetus are emphasized. With the recent advances in modern obstetrics, the possibility of antepartum diagnosis of this condition is more real today than it ever was: and in view of high fetal mortality, neonatal morbidity and increased maternal risk, the guideliness for management of this condition are suggested.
