A case of a long-standing lesion on the hand of a 64-year-old man is reported. Histology of the excision specimen showed characteristic features of mucinous syringometaplasia. The histopathological findings are described in detail including the first immunohistochemical study of this entity. Previous cases in the literature are reviewed. Mucinous syringometaplasia is a benign, presumably reactive verrucous lesion most commonly occurring on acral sites. Complete excision is curative.
