The genesis of a carpal tunnel syndrome not described before is reported. The case in question is a 45-year-old patient with a four weeks history and increasing complaints (pain on motion and at rest, positive Hoffmann-Tinel's sign, positive extension and flexion test, considerable delay in the speed of nerve conduction). During the operation a thrombosed epineural vessel was found. Histologically, it could not be stated with certainty whether this had been a thrombosed artery or vein.
