Dementia is a frequent cause of mental or cognitive impairment in elderly patients. Sometimes elderly patients are unjustly diagnosed as being demented. Two such cases are reported in this article. Both patients were referred to a geriatric outpatient clinic with a diagnosis of dementia. The first patient had auditory hallucinations as a feature of a presumed paraneoplastic syndrome. The other patient experienced auditory agnosia as a result of an as yet unrecognized stroke. To recognize supposed dementia a thorough anamnesis and clinical evaluation are a prerequisite and often indicative of the correct diagnosis.
痴呆是老年患者精神或认知障碍的常见原因。有时老年患者会被不公正地诊断为患有痴呆症。本文报告了两例此类病例。两名患者均被转诊至老年门诊,诊断为痴呆症。首例患者以幻听为特征,推测为副肿瘤综合征。另一例患者因尚未被识别出的中风而出现听觉失认症。要识别疑似痴呆症,全面的病史采集和临床评估是前提条件,且往往能提示正确的诊断。
