Through necessity, nurses have found practical ways to incorporate physicians' orders into practice. Yet, nursing care orders have not been given the same degree of importance in delivery of documentation as physician orders. Thus, care plans often become half-hearted attempts at appeasing accreditors. A nursing care plan is described that addresses some of the issues involved in the care plan debate.
