Acute retinal necrosis (ARN) syndrome is associated with members of the herpes virus family, but the mechanisms of infection remain unclear. The purpose of this study is to report a unique case of acute retinal necrosis syndrome associated with herpetic encephalitis in order to elucidate possible factors involved in herpetic central nervous system disease.
Case report.
A 64-year-old woman who developed acute herpes simplex virus encephalitis associated with bilateral acute retinal necrosis syndrome after craniotomy for resection of a suprasellar craniopharyngioma is presented. The results of lumbar puncture, magnetic resonance imaging, and ophthalmologic examination are consistent with herpetic infection. The origin of acute retinal necrosis syndrome and the association of acute retinal necrosis syndrome with encephalitis are reviewed.
After craniotomy, we hypothesize reactivation of previously latent herpes simplex virus in the area of the inferior frontal lobe and optic chiasm. Reactivated virus may have migrated to the retina by axonal transport, through the optic nerves, to produce the acute retinal necrosis syndrome.

报告一名64岁女性,在接受开颅手术切除鞍上颅咽管瘤后,发生了与双侧急性视网膜坏死综合征相关的急性单纯疱疹病毒性脑炎。腰椎穿刺、磁共振成像和眼科检查结果与疱疹感染一致。对急性视网膜坏死综合征的起源以及急性视网膜坏死综合征与脑炎的关联进行了综述。
开颅手术后,我们推测先前潜伏在额叶下部和视交叉区域的单纯疱疹病毒被重新激活。重新激活的病毒可能通过轴突运输,经视神经迁移至视网膜,从而产生急性视网膜坏死综合征。
