A patient is presented with a probable sarcoid granuloma involving the surface of the optic disc. Systemic steroid administration resulted in a resolution of the fundus changes and return of normal vision. Although ocular sarcoidosis is usually manifested by anterior segment inflammation, it is apparent that it may also produce proliferative lesions of the optic nerve.
