Evaluation of multiplex PCR utility for diagnosis of central nervous system disorders in HIV-infected patients.
Multiplex PCR assay for simultaneous detection of HSV-1, HSV-2, VZV, HHV-6, CMV, EBV, JCV, T. gondii DNA in 27 CSF samples from HIV-infected patients was used. PCR results were compared with clinical diagnosis and histopathological examination of brain obtained at autopsy.
Five of seven positive results were corresponding with clinical diagnosis. In two cases also with autopsy.
DNA of one or more pathogens was found in 6 out of 27 (22%) samples: CMV DNA in four patients, JCV DNA and T. gondii DNA in one patients each. In one case co-infections of CMV and EBV was found.
Multiplex PCR is a useful diagnostic method. LIPA-CNS test has high specificity. Lack of nesting step of the reaction and PCR positive control verification could cause a relatively low sensitivity of the test.
评估多重聚合酶链反应(PCR)在诊断HIV感染患者中枢神经系统疾病中的效用。
采用多重PCR检测法同时检测27例HIV感染患者脑脊液样本中的单纯疱疹病毒1型(HSV-1)、单纯疱疹病毒2型(HSV-2)、水痘-带状疱疹病毒(VZV)、人疱疹病毒6型(HHV-6)、巨细胞病毒(CMV)、EB病毒(EBV)、多瘤病毒(JCV)、弓形虫DNA。将PCR结果与临床诊断以及尸检时获得的脑组织病理检查结果进行比较。
7例阳性结果中有5例与临床诊断相符,2例也与尸检结果相符。
27例样本中有6例(22%)检测到一种或多种病原体的DNA:4例患者检测到CMV DNA,1例患者检测到JCV DNA,1例患者检测到弓形虫DNA。1例患者发现CMV和EBV合并感染。
多重PCR是一种有用的诊断方法。LIPA-CNS检测具有高特异性。反应缺乏巢式步骤以及PCR阳性对照验证可能导致检测灵敏度相对较低。
