To evaluate the usefulness of the vascularity in parotid glands in sonographic diagnosis for Sjögren syndrome.
Sonographic images of 72 cases of previously suspected Sjögren syndrome (including 43 actual cases) were analyzed retrospectively for the abnormal vascularity in the parotid gland parenchyma. The relationships between the vascularity and the results of sialographic, serologic, and histopathologic examinations were analyzed. We also compared the diagnostic accuracy of B-mode only with that of B-mode plus Doppler-mode.
Sjögren-positive cases showed significantly higher vascularity. As the grade of vascularity became higher, the rate of the Sjögren-negative cases became lower. The highest mean vascular score could be observed both in the initial stage and in the cavitary-destructive stage in the sialographic grades. Sensitivity and accuracy were markedly improved with vascular information.
By using vascular information, sonographic diagnosis for Sjögren syndrome can be improved.
评估腮腺血管情况在干燥综合征超声诊断中的作用。
回顾性分析72例既往疑似干燥综合征患者(包括43例实际病例)的腮腺实质异常血管超声图像。分析血管情况与涎腺造影、血清学及组织病理学检查结果之间的关系。我们还比较了仅使用B模式与B模式加多普勒模式的诊断准确性。
干燥综合征阳性病例显示出明显更高的血管情况。随着血管分级升高,干燥综合征阴性病例的比例降低。在涎腺造影分级的初始阶段和空洞破坏性阶段均观察到最高平均血管评分。血管信息显著提高了敏感性和准确性。
通过使用血管信息,可提高干燥综合征的超声诊断水平。
