About 10% of German general public have an advanced directive (representative surveys between 2002 - 2007). Surveys of patients (cancer, dialysis) show higher percentages (15 - 18%). The anticipated adherence to the advanced directive is lower in patients than in healthy persons or medical staff. To improve autonomy at the end of life not only advanced directives, but also alternative concepts such as Advanced-Care-Planning, emergency plans or stakeholders should be considered.
