Few reports about the success of high-energy extracorporeal shock-wave therapy in cases of plantar fasciitis exist, even fewer about long-term results. This study investigated results of high-energy extracorporeal shock wave therapy applied to patients with recalcitrant plantar fasciitis.
Ninety ESWT were applied to 63 patients (73 heels; 25 male and 38 female; average age 54 (29 to 77) years) from November 1999 to July 2003. All patients had plantar fasciitis for more than 6 months and failure of all non-surgical treatment for more than 3 months. A Dornier Lithotripter S, equipped with an electromagnetic shock-wave emitter was used. Routinely, 1000 shock wave impulses (frequency 2 per second, energy flux density (ED) 0.35 mJ/mm² at 10.5 kV, total dose 350 mJ/mm²) were applied per treatment. Followup was carried out 6 weeks after ESWT, then a second clinic evaluation and a final followup at an average of 73 months after ESWT by telephone.
The success of ESWT, defined as a 30% VAS reduction, was seen in 81% at 6-week followup, at 88% at last clinic followup and in 96% at final phone followup.
High-energy ESWT (0.35 mJ/mm²) was successful in the treatment of plantar fasciitis and the good short-term results seemed to be maintained over time.
关于高能体外冲击波疗法治疗足底筋膜炎的成功案例鲜有报道,更遑论长期疗效。本研究旨在探讨高能体外冲击波疗法治疗顽固性足底筋膜炎的疗效。
1999 年 11 月至 2003 年 7 月,我们对 63 例(73 足跟;25 例男性,38 例女性;平均年龄 54 岁[29 至 77 岁])患有足底筋膜炎且经非手术治疗 3 个月以上无效的患者,共进行了 90 次 ESWT。采用 Dornier Lithotripter S,配备电磁冲击波发射器。常规情况下,每次治疗应用 1000 次冲击波脉冲(频率为每秒 2 次,能量通量密度(ED)为 10.5kV 时为 0.35mJ/mm²,总剂量为 350mJ/mm²)。ESWT 后 6 周进行随访,然后在第 6 周进行第二次临床评估,平均在 ESWT 后 73 个月通过电话进行最终随访。
ESWT 成功的定义为 VAS 评分降低 30%,在 6 周随访时为 81%,在最后一次临床随访时为 88%,在最终电话随访时为 96%。
高能 ESWT(0.35mJ/mm²)治疗足底筋膜炎有效,且短期疗效良好,长期疗效似乎也能维持。
