Patient competence is a prerequisite to fulfilling the primary ethical principle of patient autonomy or self-determination. This article defines the legal designation of incompetence and describes the components of the assessment of a patient's decision-making capacity. These assessments occur within the context of relationships, the most influential of which is that between the nurse and patient. Attention to the nurse-patient relationship promotes patient autonomy, patient advocacy, and decision making based upon the patient's values--which is, ultimately, ethical health care.
患者具备行为能力是践行患者自主或自我决定权这一首要伦理原则的前提条件。本文界定了无行为能力的法律认定,并描述了评估患者决策能力的构成要素。这些评估是在各种关系的背景下进行的,其中最具影响力的是护士与患者之间的关系。关注护患关系可促进患者自主、患者权益维护以及基于患者价值观的决策制定,而这最终就是合乎伦理的医疗保健。
