Abstract

Purpose To compare dynamic contrast material-enhanced (DCE) magnetic resonance (MR) imaging parameters between metastatic bone lesions with and without the epidermal growth factor receptor (EGFR) mutation in consecutive patients with primary non-small cell lung cancer (NSCLC). Materials and Methods This study was approved by the institutional review board. Forty-seven patients with NSCLC and a confirmed EGFR mutation status (27 patients were positive and 26 were negative for EGFR mutation), who underwent DCE MR imaging for bone metastases between November 2012 and March 2016, were included in this study. Two radiologists independently analyzed the volume transfer constant (K), reflux rate (k), and volume fraction of the extravascular extracellular matrix (v) using image processing software. Intergroup comparisons of the mean measured parameters were performed with the Mann-Whitney U test. Interobserver agreement was calculated with the intraclass correlation coefficient. Results There was a high level of agreement between the two reviewers for all three parameters (intraclass correlation coefficient = 0.95 for K, 0.97 for k, and 0.91 for v). K was significantly higher in the EGFR mutation-positive group (P = .039 for reviewer 1, P = .032 for reviewer 2). k was also higher in the EGFR mutation-positive group but showed statistical significance only in the evaluation performed by one reviewer (P = .048 for reviewer 2, P = .058 for reviewer 1). No significant difference was observed in v (P = .873 for reviewer 1, P = .889 for reviewer 2). Conclusion The differences in the DCE MR imaging parameters between metastatic bone lesions with and without EGFR mutations in primary NSCLC may be attributed to differences in the vascular structure related to angiogenesis stimulated by the activation of the EGFR signaling pathway. 目的 在连续的原发性非小细胞肺癌(NSCLC)患者中,比较伴有和不伴有表皮生长因子受体(EGFR)突变的转移性骨病变的动态对比增强(DCE)磁共振(MR)成像参数。

材料与方法 本研究经机构审查委员会批准。纳入 2012 年 11 月至 2016 年 3 月间因骨转移而行 DCE MR 成像检查且 EGFR 突变状态已明确的 47 例 NSCLC 患者,其中 27 例 EGFR 突变阳性,26 例 EGFR 突变阴性。两位放射科医生使用图像处理软件分别独立分析容积转移常数(K)、回流率(k)和血管外细胞外基质体积分数(v)。采用 Mann-Whitney U 检验对组间平均测量参数进行比较,采用组内相关系数评估观察者间一致性。

结果 两位观察者对所有三个参数的一致性均较高(K 的组内相关系数为 0.95,k 的组内相关系数为 0.97,v 的组内相关系数为 0.91)。EGFR 突变阳性组的 K 值显著高于 EGFR 突变阴性组(观察者 1:P =.039;观察者 2:P =.032)。EGFR 突变阳性组的 k 值也较高,但仅在一位观察者的评估中具有统计学意义(观察者 2:P =.048;观察者 1:P =.058)。v 值无显著差异(观察者 1:P =.873;观察者 2:P =.889)。

结论 在原发性 NSCLC 中,伴有和不伴有 EGFR 突变的转移性骨病变的 DCE MR 成像参数的差异可能归因于与 EGFR 信号通路激活刺激的血管生成相关的血管结构的差异。
