The treatment for giant cutaneous squamous cell carcinoma (cSCC) is challenging. Here, we reported that a 90-year-old patient with giant cSCC was successfully treated through combination of surgery and photodynamic therapy (PDT). Due to the advanced patient age, large tumor area, and deep infiltration, radical surgery cannot be performed initially. After in situ resection, five rounds of topical 5-aminolevulinic acid PDT were continuously performed. The surgical wound was healed five months after resection. Seven months after initial resection, the residual tumor was resected by a radical surgery. The patient was followed up for six months after the radical surgical resection and showed no tumor recurrence. This case suggested that initial utmost resection for main body of tumor, decreasing the tumor boundary by PDT treatment and the sequential treatment regimen of radical surgery can be an effective therapeutic schedule for refractory and giant cSCC.
巨大多形性角质细胞癌 (cSCC) 的治疗颇具挑战性。在此，我们报告一例 90 岁巨大多形性角质细胞癌患者，成功接受手术联合光动力疗法 (PDT) 治疗。因患者高龄、肿瘤面积大、浸润深，初始无法行根治性手术。行局部切除后，连续进行了 5 轮 5-氨基酮戊酸 PDT。切除后 5 个月，手术创面愈合。初次切除 7 个月后，行根治性手术切除残余肿瘤。根治性手术切除后，患者随访 6 个月，未见肿瘤复发。该病例提示，对于难治性和巨大 cSCC，初始行主体肿瘤最大限度切除，PDT 治疗缩小肿瘤边界，序贯根治性手术的治疗方案可能是一种有效的治疗策略。
