Local anesthetic infiltration is used widely for post-operative analgesia in many situations. However the effects of local anesthetics on wound healing are not demonstrated clearly. This study planned to evaluate the effects of lidocaine, prilocaine, bupivacaine and levobupivacaine on wound healing, primarily on wound tensile strength and on collagen ultrastructure.
This study was conducted on male Sprague Dawley rats. On days 0, 8th, 15th, and 21st, all animals were weighed and received a preincisional subcutaneous infiltration of 3 mL of a solution according the group. Control saline (C), lidocaine (L) 7 mg.kg, prilocaine (P) 2 mg.kg, bupivacaine (B) 2 mg.kg and levobupivacaine (LVB) 2.5 mg.kg. The infiltrations were done at the back region 1.5 cm where incision would be performed at the upper, middle and lower part along the midline, under general anesthesia. Wound tensile strengths were measured after 0.7 cm × 2 cm of cutaneous and subcutaneous tissue samples were obtained vertical to incision from infiltrated regions. Tissue samples were also obtained for electron microscopic examination. Evaluations were on the 8th, 15th and 21st days after infiltration.
There was no difference between groups in the weights of the rats at the 0th, 8th, 15th and 21st days. The collagen maturation was no statistically different between groups at the 8th and 15th days. The maturation scores of the B and L groups at the 21st day was significantly lower than the Group C (1.40, 1.64 and 3.56; respectively). The wound tensile strength was no statistically different between groups at the 8th and 15th days but at the 21st day the Groups B and LVB had significantly lower value than Group C (5.42, 5.54 and 6.75; respectively).
Lidocaine and prilocaine do not affect wound healing and, bupivacaine and levobupivacaine affect negatively especially at the late period.
局部麻醉药浸润在许多情况下被广泛用于术后镇痛。然而,局部麻醉药对伤口愈合的影响尚未明确证实。本研究旨在评估利多卡因、丙胺卡因、布比卡因和左旋布比卡因对伤口愈合的影响,主要是对伤口抗张强度和胶原超微结构的影响。
本研究以雄性斯普拉格-道利大鼠为实验对象。在第0、8、15和21天,对所有动物称重,并根据分组情况在切开前于皮下注射3毫升溶液。对照组注射生理盐水(C),利多卡因(L)7毫克/千克,丙胺卡因(P)2毫克/千克,布比卡因(B)2毫克/千克,左旋布比卡因(LVB)2.5毫克/千克。在全身麻醉下,于背部距中线1.5厘米处进行浸润,沿中线在上、中、下部分做切口。从浸润区域垂直于切口获取0.7厘米×2厘米的皮肤和皮下组织样本后,测量伤口抗张强度。还获取组织样本用于电子显微镜检查。在浸润后的第8、15和21天进行评估。
在第0、8、15和21天,各组大鼠体重无差异。在第8天和15天,各组之间胶原成熟度无统计学差异。在第21天,B组和L组的成熟度评分显著低于C组(分别为1.40、1.64和3.56)。在第8天和15天,各组之间伤口抗张强度无统计学差异,但在第21天,B组和LVB组的值显著低于C组(分别为5.42、5.54和6.75)。
利多卡因和丙胺卡因不影响伤口愈合,而布比卡因和左旋布比卡因尤其在后期有负面影响。
