Abstract

BACKGROUND

Bartonella species are intracellular bacteria capable of producing several diseases in humans. The three most common and wellknown diseases are cat scratch disease (CSD), caused by B. henselae, trench fever, caused by and Carrion's Disease, caused by . Signs and symptoms are very different and aspecific: Fatigue, fever, headache, lymphadenopathy, malaise, loss of weight. No data exist to support guidelines' recommendations to decide which drugs should be optimally used and how long they should be administered. Therefore, a marker of treatment response is needed to guide treatment strategies.

METHODS

We report herein three cases in which a species specific Reverse-Transcriptase Polymerase-Chain-Reaction (RT PCR) developed in-house was performed and compared to serology in order to make diagnosis and to evaluate treatment response.

RESULTS

Our species-specific RT PCR seemed to play a fundamental role both in diagnosis and treatment. Moreover, a discrepancy with the serology results was found.

CONCLUSION

Further studies are necessary to validate these results and elucidate what is the best treatment for this pleomorphic disease. However, in absence of clear guidelines, RT PCR may be useful to orientate kind of treatment ad its duration.

背景

巴尔通体属细菌是细胞内细菌,可导致人类多种疾病。三种最常见且广为人知的疾病是由汉赛巴尔通体引起的猫抓病(CSD)、由五日热巴通体引起的战壕热以及由杆菌状巴尔通体引起的卡里翁病。其体征和症状差异很大且缺乏特异性:疲劳、发热、头痛、淋巴结病、不适、体重减轻。目前尚无数据支持指南中关于应最佳使用何种药物以及用药时长的建议。因此,需要一种治疗反应标志物来指导治疗策略。

方法

我们在此报告三例病例,其中运用了自行研发的种特异性逆转录聚合酶链反应(RT PCR),并与血清学检测进行比较,以做出诊断并评估治疗反应。

结果

我们的种特异性RT PCR似乎在诊断和治疗中都发挥了重要作用。此外,还发现了与血清学结果的差异。

结论

需要进一步研究以验证这些结果,并阐明针对这种多形性疾病的最佳治疗方法。然而,在缺乏明确指南的情况下,RT PCR可能有助于确定治疗类型及其持续时间。
