Abstract

OBJECTIVE

To analyse the diagnostic performance of MTB/RIF assay for the diagnosis of pulmonary tuberculosis and detection of rifampicin resistance using sputum samples.

STUDY DESIGN

Observational cross-sectional study.

PLACE AND DURATION OF STUDY

Provincial TB Reference Laboratory (PTRL), Hayatabad Medical Complex, Peshawar, Pakistan, from January to October 2015.

METHODOLOGY

A total of 268 participants were consecutively enrolled in the study after meeting the inclusion criteria. Their sputum samples were collected and processed by N-acetyl-L-cysteine-sodium hydroxide (NALC-NaOH) method and GeneXpert MTB/RIF assay.

RESULTS

This study determined the overall sensitivity and specificity of MTB/RIF assay, it was 92.4% (86/93) and 97.1% (138/142), respectively. The sensitivity was 98.4% (60/61) in culture proven smear positive samples, whilst sensitivity in culture proven smear negative samples was 93.7% (30/32), using culture as reference standard.

CONCLUSION

GeneXpert MTB/RIF assay could greatly improve early diagnosis of PTB in smear negative cases as well as multidrug resistant tuberculosis.

目的

分析采用痰液样本的MTB/RIF检测法对肺结核的诊断性能及利福平耐药性的检测情况。

研究设计

观察性横断面研究。

研究地点及时间

2015年1月至10月,巴基斯坦白沙瓦哈亚塔巴德医疗中心省级结核病参比实验室(PTRL)。

方法

共有268名符合纳入标准的参与者连续纳入本研究。收集他们的痰液样本,采用N-乙酰-L-半胱氨酸-氢氧化钠(NALC-NaOH)法和GeneXpert MTB/RIF检测法进行处理。

结果

本研究确定了MTB/RIF检测法的总体敏感性和特异性,分别为92.4%(86/93)和97.1%(138/142)。以培养为参考标准,培养确诊的涂片阳性样本中的敏感性为98.4%(60/61),而培养确诊的涂片阴性样本中的敏感性为93.7%(30/32)。

结论

GeneXpert MTB/RIF检测法可极大地提高涂片阴性病例及耐多药结核病中肺结核的早期诊断率。
