CASE
A 19-year-old man with severe hemophilia A underwent open surgical synovectomy to treat recurrent hemarthrosis of the knee. He developed symptoms and signs of acute compartment syndrome (ACS) despite receiving full-dose factor replacement. Arthrotomy was performed twice, and massive hemarthrosis was evacuated. The symptoms subsided after supplementing clotting factor with prednisone.
CONCLUSIONS
ACS may complicate hemophilia. Diagnosis is mainly based on clinical judgment. Proper factor replacement is the mainstay of treatment. Surgery is reserved for patients with no improvement following factor administration.
病例
一名19岁的重度甲型血友病男性患者接受了开放性手术滑膜切除术，以治疗膝关节反复出血。尽管接受了全剂量的凝血因子替代治疗，他仍出现了急性骨筋膜室综合征（ACS）的症状和体征。进行了两次关节切开术，并清除了大量关节积血。补充凝血因子并加用泼尼松后症状缓解。
结论
ACS可能是血友病的并发症。诊断主要基于临床判断。适当的凝血因子替代治疗是主要的治疗方法。对于在给予凝血因子后无改善的患者才考虑手术治疗。
